Social Worker - Children & Family Support

The purpose of the Social Worker- Children and Family Support is to determine, implement and coordinate Children’s Long Term Support social services to children with disabilities and their families who qualify, for the Sauk County Department of Human Services.
Essential Duties
- Assess children with disabilities for functional and financial eligibility for the Children’s Long Term Support Waiver (CLTS) and Children’s Community Options Program (CCOP).
- Develop an Individual Service Plan (ISP) with families that identify family outcomes and services to meet those outcomes.
- Refer child/family to other agencies, intervene in crisis situations, monitor service delivery and act as an advocate for child and family.
- Monitor funding sources and track costs to stay within budget and maintain quality compliance.
- Assess child health and safety, report child abuse and neglect, and complete Critical Incident reports and submit to the state Children’s Service Section.
- Assist children/families with transitions back to the community from institutions and/or hospitalizations.
- Coordinate smooth transition from the children’s services to adult services; initiate referrals to the Aging and Disability Resource Center; and initiate and complete guardianship court process when appropriate.
- Enter information into electronic systems for billing, documentation and state reporting.
- Participate in child find and other community awareness activities.
- Provide unit case management in absence of assigned Service Coordinator/Social Worker.
- Arrange or provide client transportation for disability related services.
- Participate on Comprehensive Community Services and Families Come First teams as needed.
- Assist in program reviews/audits.
- And other duties as assigned.
Required Working Hours
Standard working hours are Monday through Friday, 8:00 am until 4:30 pm. Please note these hours are subject to change and additional hours may be needed or required. Compensation for additional hours beyond normal working hours are subject to applicable state and federal regulations.
Minimum Training and Education
Required:
Bachelor’s degree in Social Work, Counseling, or another related field
2 years of related experience
Licenses/Certifications:
Valid Wisconsin State Motor Vehicle Operator’s License
Obtain a Wisconsin State Social Worker License within 6 months of start date
Become Comprehensive Community Services (CCS) provider Certified within 6 months of start date
Preferred:
Wisconsin State Social Worker License
Disability case management experience
Any combination of education and experience that provides equivalent knowledge, skills and abilities may be considered.
